The Cardiac Connection offers individualized and comprehensive home health care to our clients. A Registered Nurse will conduct a preliminary evaluation of the type of services needed in your home. Family members, physicians and other health care professionals will work together to develop a customized plan of care. The care plan is reviewed weekly, it includes details about the services to be provided, its frequency and duration of care needed and is updated to meet your changing needs.

Occupational Therapy
As an Occupational Therapist (OT) and Occupational Therapist Assistant (OTA) the main focus is the upper body as well as “Occupational activates” that the medical world calls Activities of Daily Living (ADL’s). ADL’s consist of everyday things that some people may need help with. For example, showering, getting dressed, writing etc. When most people hear occupational therapy they think that the just do the upper body exercises but, it is so much more. OT combines upper body training with ADL’s to make a complete circle of occupational health.
Furthermore, OT looks for modifications that can be implanted in the home to make it easier for patients to get around. For example, do patients need a bedside rail, grab bars, raised toilets, and ramps in front of the house? OT ensures that all of those things and more are put in the home in order to avoid accidents or help make living in the home more comfortable and accessible. “What I like about TCC…we have the time to spend with the patient,” says TCC COTA. It all boils down to…what does this patient need to take care of themselves and live safely in their home.
Physical Therapy
As a Physical therapist (PT) and Physical therapist Assistant (PTA), Exercise is the main thing that we do here at TCC. Physical therapy works on the lower body and helps the patient perform exercises such as sitting exercises (for our geriatric(senior citizens) community) Ankle pumps, long arc, and short arc quad. Hip flexions, AD ductions using ball squeezes. Adding resistance as the patients get stronger.
Over time as the patient gets stronger, we begin to move from sitting exercises to standing exercises. Standing exercises include but are not limited to heel-toe raises, kicking to the side, kicking to the back, hip extensions, hip flections, Hamstring curls (bending their knees), and squats. For patients who have trouble standing up there are laying down exercises such as bridges, trunk rotation, pelvic tilt, there is also marching and laying down supine.
These are all exercises that PT’s and PTA’s do everywhere. What really separates TCC from the rest is the “IT” factor that we call care. Most PT’s and PTA’s are too busy trying to get their notes or perform some other documentation needed. At TCC we take the time out to get to know our patients and perform timely documentation. “I like to spend time with my patients…It’s the little things that count” Says a PTA for TCC.

As a skilled nurse for a Cardiac Home health care company, their job is to be the point of contact between the patient and the doctor.
It starts off with a comprehensive assessment but, leads into other things as Medication management, Labs, Low sodium diet and hypertension education, CHF Maintenance and heart-healthy teaching, maintain PICC lines; perform cardiopulmonary assessments, and focus on bettering cardiovascular health.
Our nurses pose as the liaison between the Doctors office and the patient. Because of this, they are well versed in Left ventricular assistive devices (LVAD) and Central lines.
Our nurses work with the entire care team including but not limited to doctors, nurses, and caregivers for our patients. That way the patient receives the best care possible and everyone apart of the patients care team is on the same page. The difference between us and them is that the patient is part of the plan of care.
